Create a Form. This form will be the basis of your survey.
Add Questions to your Form. These are the questions that attendees will be able to respond to.
To implement the survey, you must first enable requests and select a request profile. The request profile designates who will receive the survey responses and will allow you to apply the survey to your item. Ensure that the request profile is set to your event's team.
Apply the form to the item you wish to place it in.
